All candidates who have qualified the Recruitment Test held on 29.06.2014 for the subject post
are requested to furnish self attested photocopies of documents/certificates in support of their claim for
the post alongwith Online Recruitment Application (ORA).

1. Matriculation/Xth Standard or equivalent certificate indicating date of birth OR
Matriculation/Xth Standard or equivalent Marksheet issued by Central/State Board indicating
date of birth
2. Caste certificate from a Competent Authority in prescribed format as per advertisement
indicating clearly your (i) Caste (ii) the Act under which the Caste is recognized as SC/ST/OBC
and (iii) the village/town you are ordinarily a resident of. The Issuing Authority of the Caste
certificate should be as per the Advertisement. [for SC/ST/OBC candidates]
3. Master’s Degree/Degree along with Mark Statements/Marksheets pertaining to all the academic
years/semesters in support of your Educational Qualifications as per the Advertisement No.
11/2013 (Item No. 19, Vacancy No. 13081119210). (In case Master’s Degree/Degree is yet to
be conferred upon you, then provisional Master’s Degree/certificate along with marks
statements pertaining to all the academic years should be furnished as proof of educational
qualifications.
4. Certificate of Government Servant (for candidate claiming age relaxation as Govt. Servant) in
the prescribed format as per the Advertisement.
5. Certificate of Physically Handicapped Persons in the prescribed format as per the Advertisement
(for PH candidates only)
6. An Affidavit if there is difference in either name/father’s name or spelling of name/father’s name
in all the Certificates with reference to Online Recruitment Application (ORA).
The format of Caste, Govt. Servant, PH etc. is available on Commission’s Website
www.upsc.gov.in.
Important: Originals of the above certificates are required to be produced at
the time of interview.
You are requested to furnish the self attested photocopies of the above documents/certificates
alongwith Online Recruitment Application (ORA), mentioning Roll Number on each
documents/certificates, by Hand/Speed Post to the Under Secretary (SPC-I), Room No. 3, UPSC,
Shahjahan Road, New Delhi-69 within 15 days in any case not later than 25.08.2014. You are also
requested to write the name of Post on the Envelopes. Date & time of Interview for the above posts will
be intimated later on through e-mail.
